Mālama Ola Athletic Trainers (ATs) provide care to student-athletes in collaboration with the KS Student Health Services Director, team physician(s), and/or the student’s private physician(s), following their training and state regulations. Injuries or illnesses beyond the scope of athletic training will be referred to the student’s healthcare provider or the nearest emergency facility.
Services provided by ATs include primary care, injury and illness prevention, wellness promotion and education, emergent care, examination and clinical diagnosis, therapeutic intervention and rehabilitation of injuries and medical conditions.
Physical evaluation requirements: To participate in sports, student-athletes are required to have a physical evaluation every 13 months (12 months with a one-month grace period). We recommend having the KS Physical Evaluation form completed during your child’s annual well-child visit. Once the form is completed by your child’s healthcare provider, please upload it to your child’s profile in the Mo‘omō‘ali Olakino (EHR) Parent Portal. The KS Physical Evaluation form and detailed instructions for logging in and uploading documents to the EHR Parent Portal can be found on the Forms and Checklist page.
Health insurance: Please ensure your child has active health insurance, as this is required for participation in school sports.
For additional information, please see your campus’ student and parent handbook.
